Sexually transmitted disease in Chile, a bigger problem for Midwives and obstetricians

AIMS:

Sexually transmitted diseases have historically been a common health problem.
In addition to causing damage to the site of infection, many of this have systemic manifestations is important that the midwife have to check, like its sintomatology, its treatment and its complications.
With this objective we performed this review  of the main aspects of epidemiology, pathogenesis, diagnosis, symptoms and treatment of diseases most prevalent sexually transmitted Chile.

Methods:

Epidemilogy Methods

Since 1989, the disease shows a sustained decline in rates of 64.9 per 100000, and from 1989 to 1998 this decline has been significant, probably due to increased use of condoms and other prevention methods like sexual education.
Actually prevention methods is released by health professionals, because the only way to reduce the sexually transmitted disease is with a good counseling in this aspect, and midwives, obstetricians and gynecologists are responsible  for the health of all women of reproductive age as both climacteric and menopause

Results

Vulvovaginal infections constitue one of the most frequent and important problems of morbidity in paedriatric gynaecology. We present the experience of the Department of Paedriatric Gynaecology, Dr. Luis Calvo Mackenna Hospital and the activities care and control in specific sexually transmitted vulvovaginal infections that have been implemented in the last decade.
Activities Care and Control Unit of Sexual Health (UNACESS)

   Specialized outpatient morbidity in STI
   Control of disease specialist STI
   Sexual health consultation
   Sexual health check
Care for victims of sexual violence Personalized Education
   Counseling for STI risk management
Pre and post test counseling for HIV ELISA
   Management (s) partner (s) / contact (s) sexual (es)

The implementation of these standards of care have declined the impact that can cause sexually transmitted diseases in paediatric.

Discussion:
Address this need and taking into account the implementation of Health Reform, the law and technological advances convened a multidisciplinary team, consisting of a group of experts with vast experience and management in the field, to prepare This "Statement of Management and Treatment of Sexually Transmitted Infections in Chile".

These health reforms were implemented in the Luis Calvo Mackenna Hospital successfully, since the application of these techniques, care and control of sexually transmitted infections in the field of gynecology and obstetrics have allowed the control of morbidity and mortality from these infections reducing mortality rates of sexual infections.

Conclusions:

Management and care of people with sexually transmitted infections are relevant to the performance of midwives at public health, since the implementation of measures to reduce mortality and morbidity from these causes must be studied previously, and obstetricians should be aware at the time of use control strategies having a transverse view for the evolution of the patient to be successful and achieve better health standards.

Cervical Cáncer, disturbing reality

The Cáncer, is a disturbing reality for all the people in the world, but the real meaning of this word lies in the context of the word, because people relate with dead and this is the biggest fear of humans.
The cervical Cáncer is a bigger problem that affect thousands of women in the planet, and in Chile it is the principal problem for us, the Midwives and Obstetricians.
Which is it cause? well, cervical cáncer is caused by human papilomavirus or HPV, of which only 2 strains are carcinogenic, the strain 16 and 18. This strains of the virus attacks squamous or glandular epithelial cells that line the cervix (The cervix is the narrow portion of the womb where it joins with the top of the vagina).
Which are the signs and symptoms? Cervical cancer may not cause any symptoms at first, but later, you may have pelvic pain or bleeding from the vagina. It usually takes several years for normal cells in the cervix to turn into cancer cells.
Prevention of this cáncer: A responsible sexuality, with the use of condoms offer protection against this cáncer, so the women should be serius in their sexual health, because this virus is highly contagious through of fluids and skin in the mucous membranes (oral and genital).
The HPV vaccine is other prevention method, but is very expensive and only women that not have sex can use.

The treatment consists in surgery (local excision in the womb) in early stages and hysterectomy (remove all womb and part of the vagina) with chemotherapy or radiotherapy in advanced stages of the disease.

This is a big trouble for us, and we are the responsible for sexual health education of women, so we have a lot of work to do.
Education is the key :)
